There used to be an AMSAT "QSL Insert" along the similar lines of your
thinking.

The old version is still on-line at:
http://www.amsat.org/amsat/amsat-na/membership.html and then scroll down
near the bottom of the page to QSL Insert.

But that was 2003 so its likely a few things on that page are now
out-of-date.  Perhaps it could be used as a model for an updated QSL Insert?
