There have been numerous articles in the magazines (QST) regarding
the isolation transformer and opto isolator approaches.
The best "build your own" solution that has a web site is
"Buck" Rogers, W4ABT's interface home page.

Choose your Radio, click on "View Drawing". You get a nice schematic
(which you can print out) and parts list.
While Buck does sell a kit and offer assembled units for handicapped
amateurs,
I prefer the individual part procurement method, which he also sells.
Yes, you can buy just the PC board, transformer or enclosure.
