In general, it is unnecessary, and bad form, to include
the index.htm or index.html part of the name. It is the default,
and omitting it means that the page author can change the
name if he needs to, without breaking links.

Other valid examples are index.shtml (with server-side includes
for Apache and some other web servers), index.phtml (includes PHP
code), or even index.jhtml with Java.
