Any multiple access scheme needs a congestion avoidance algorithm to
keep this from happening. Ethernet retransmissions are subject to the
random exponential backoff function. In TCP/IP, something similar
happens when TCP times out waiting for an ACK. Algorithms are
available for every kind of contention access scheme, including CDMA,
and they do work when they are used.

Conventional amateur radio operation suffers from having ill-defined
congestion avoidance algorithms that are (poorly) implemented in
wetware.
