d.c. insertion moduals

 does anyone remember the cap and choke values for a coaxial d.c.
insertion modual for use to 500mhz? im powering my preamps via
coax now. (or want to)
 i cant find the data. i had it somewhere. anybody got 2 to sell 
cheap?
